    We support and care for adults living with a learning disability, who may also have a diagnosis of autism, in specialist supported living, residential services and hospitals. Our aim is to deliver effective and outcome based services within person centred, professional care environments, supporting people to have the most choice and independence possible. We set high standards for ourselves in the delivery of care, and most importantly, we put the people we support at the heart of everything we do.

    Working with the Service Manager, the Care Coordinator  will be responsible for the appropriate coordination and administration of the care delivery team to ensure provision of commissioned services to the people we support and be the first point of contact for day to day operational requirements of care team.




    So what qualifications and attributes do you need?

    • We welcome all applicants that feel they would be able to make a positive difference to people's lives
    • You would need to be compassionate, understanding and trustworthy
    • Health and Social Care QCF or SVQ would be beneficial
    • Previous experience would be beneficial, but not essential

  • Supported Living is an important step towards independence. We know how important it is for people to have the right support so they can live the life they want to live.     Working with Registered Housing Providers, we support people to find their own home that suits their needs. This might include large living space, a garden, a wet room or bathroom to accommodate reduced mobility, as well as local community facilities. We ensure that their needs and preferences are met and that people are able to easily do the things they want to do.

     

    We are here simply to support people to achieve their goals and dreams, whatever they may be. Our aim is to help the people we support to build their skills and confidence, so they are able to live as independently as possible
